For those not in the loop, The Final Puzzle is a challenge in Resident Evil Requiem that has been stumping members of the community since launch- it's only description was a cryptic message about "Let the sweet pair hear the voice".
Fans had gone to dramatic lengths sharing intel, even starting a subreddit called r/TheFinalPuzzle dedicated to pooling resources on how to solve Resident Evil's most cryptic challenge yet.
Finally, user Gengar Collects became the first user to announce their completion- saying that they'd stumbled into some of the steps entirely by accident.
Since then, members of The Final Puzzle community also reverse engineered the steps- with fans realizing the main key to the puzzle wasn't so much The Girl's severed hand, but rather the item called Marie's Doll.
Hilariously enough, this isn't the first time Capcom has told the grand council of survival horror fans to put their heads together- the demo for Resident Evil 7 also featured a similar cryptic puzzle required to unlock the true ending.
Resident Evil Requiem The Final Puzzle Solution (Spoiler Alert)
Interestingly enough, you won't be able to complete The Final Puzzle on your first playthrough- players will instead need to complete one half of the puzzle in one playthrough before completing the other half on a second run.
The Sweet Pair in the description refers to Marie and Emily, though it might be hard to get Marie to listen to anything given she's been turned into The Girl.
The steps to unlock The Final Puzzle in Resident Evil Requiem are as follows:
- In the East Wing toilets, flush them a total of 8 times.
- At the Meat Processing plant, wait between 15 to 20 minutes to let the bodies be fed into the grinder. You'll need to wait next to the typewriter, and the trigger you're actually looking for is 115 un-bagged bodies.
- Afterwards, during the fight on the conveyor belt, don't kill any of the zombies with a gun- instead let them be crushed by the grinder, either by stunning them and shoving them into it or simply running around them.
- After Grace climbs out of the blood pool, hit the lever next to it to drain the Blood pool- if you've done the steps correctly, you should get a notification calling it a "Torrent of Blood".

It's important to note that the toilet order doesn't seem too important- if you get a notification saying it's a Ton of Blood prior to flushing the toilets, flushing them before you escape the clinic should still work.
These steps come to fruition in the next segment with the underground lab- while Grace is escaping with the pilot key, having done the steps correctly will spawn Marie's Doll at the foot of the staircase. Take this item and leave it in your item box for the rest of the game, completing it normally.

The second half of The Final Puzzle is completed in your next run- after escaping the basement with Emily, you'll want to pick up The Girl's hand, which can be analyzed to give you a cryptic code. It's worth mentioning just how intensely the community has been trying to decipher this code- keys to its solution are hidden in areas like the blood analysis machine itself as well as the mysterious hourglass in Victor's office.
Now, while carrying Emily and Marie's Doll (taken from your Item Box), you'll want to head to the East Wing and go to the safe in the Security Director's room. Input the following code (derived from analyzing the Hand and extra clues):
Sun Sun Star
Sun Moon Star
Sun Moon Sun
Moon Star Moon
Doing so will trigger a child's laughter. If you have both the doll and you're carrying Emily, the game will give you a popup to let you know you've completed the Final Puzzle.

The puzzle itself is hinted at many times in the in-game files: the number of times for the toilet flushing is written on the walls in one of the reports, while the number of corpses is in another file explaining how they turned Marie into The Girl.
Given that most of the steps have an in-universe explanation- you're simply unclogging the doll from the drains only for it to show up as you escape the lab, it's a cool send up to one of the most compelling levels in the title.
